Job Description Summary Full Time Patient Care Coordinator position Job Responsibilities Prioritizes notes in Case Management and Hot Box.
Completes physician order in the morning and submits to the RNCM for co-signing prior to the end of day.
Assists all patients through Standards healthcare system by acting as a patient advocate.
Completes case conferences on all patients that are assigned to their case management team and coordinates with all disciplines involved in each individual patient care.
Reviews all routine visits which include would care and injections.
Assure documentation is correct and demonstrates best practice.
Reviews communication notes for correct information and ensure that the note is a comm note and does not require an order instead.
Documents any communication with patients/medical providers as required in communication notes.
Follow up on F2F, collaborates patient appointments as appropriate, coordinates with intake and orders processors.
Receives daily nursing reports and reports any pertinent changes to their RNCM and the Director of Clinical Operations/VP of Clinical Operations Coordinates orders from physicians and obtains orders from physicians to support continuity of care.
Collaborates with scheduler to ensure visits are scheduled including PRN visits.
Process notes in QA manager box daily to ensure that documents are completed, and billing can be processed.
Qualifications Experience with MS Word, Excel and PowerPoint Home Health experience is preferred Excellent written and verbal communication The ability to maintain a high level of confidentiality Must be able to demonstrate team behavior and must be willing to promote a team-oriented environment.
