Registered Nurse (Care Coordinator Home Telehealth)
This is a bargaining unit position.
Tour of Duty:
Monday - Friday, 7:
30 AM to 4:
00 PM.
Tour of duty may be subject to change to meet the needs of the mission.
The Staff Nurse is a professional nurse at an intermediate level of development.
As progression is made in the nursing profession, it is expected that leadership skills and competencies will be sustained with expertise and a broad-based clinical knowledge to effectively implement health promotion, management of acute and chronic illness, and care of the dying.
The nurse is obligated to ongoing, comprehensive evaluation and improvement of clinical practice and health care delivery systems to meet the needs of patients in the CCHT Program, including individuals, significant others, and/or communities.
Practice is based on current and anticipated trends in health care.
The Staff Nurse in the CCHT program demonstrates leadership in delivering and improving holistic care through collaborative strategies with others.
The incumbent is directly responsible to the Care Coordination Home Tele-Health Nurse Manager.
Required to coordinate and manage a panel of patients to assure that care is timely, appropriate, of high quality and cost effective.
He/She will work with specialists, primary care providers, other healthcare professionals, prosthetics, DSS, ancillary services and patients, patient families or caregivers.
He/She provides professional assessment, planning and coordination of care to evaluate and initiate changes to ensure the veteran of timely, consistent, and appropriate care.
Will identify problems and areas of concern, develop and initiate a plan of action, and evaluate effectiveness of actions.
Each veteran and his/her family will be treated individually, on a case by case basis with the needs of each situation carefully evaluated.
The CCHT Staff Nurse will establish methods to monitor patients' progress and maintain appropriate documentation.
The place of duty will be at the Temple VAMC, though travel to other VA facilities within CTVHCS, including Waco, Austin, Bryan, Brownwood, Palestine, Cedar Park is possible for training, education, and facilitation of nursing care for veterans.

Estimated Salary: $20 to $28 per hour based on qualifications.
